Midland Flyers 2023/24 Season

Permission to Skate Forms

  • All players that are 'minor hockey age' must obtain a permission to skate form and submit to us before stepping on the ice. 

  • If you are carded to another team - you must obtain a permission to skate form from your carded team.

  • please reach out to your home centre if there is any uncertainty

Players under 18

  • if the player is under 18 - they must provide parental information on the form below.

Fees and Forms

  • The cost of the camp is $75

  • after submitting the form, please etransfer the fee of $75 to eplett9994@rogers.com and ensure to include the players name in the memo field.

  • Someone will email you with an acceptance once the registration and funds are received.

Saturday, August 26th              3:00pm to 5:00pm   Midland Arena

Main Camp Registration
